Yep, you've already listed the 4 major restricted regions.
Some more rare cases:
- Some of the Amazon keys were region locked to North America only.
- There are cases when certain games can not be activated in certain countries (Germany, for instance).

Keys from Gamestop can be locked to NA as well. Japan is in the same boat as Germany in some cases. Ugh, trying to remember all these locks is hard XD.

I had a "Argentina, Bolivia, Brazil, Bahamas, Belize, Chile, Colombia, Costa Rica, Ecuador, Guatemala, Guyana, Honduras, Mexico, Nicaragua, Panama, Peru, Paraguay, Suriname, El Salvador, Uruguay or Venezuela.", but activated it a few days ago so can't take a screenshot. i don't have screenshot for it but some of square enix games are (ru/cis/in) region blocked russia/cis/India.
gta 5 has a India only region lock i believe

There are few games that are treated differently, mostly games published by Bethesda (for example Rage wasn't even listed in polish steam store for like 2 years and Dishonored have this notification on polish store page: "Notice: This game is not giftable in your territory."), however Skyrim seems to be free of region locks. But for most part, region locks for Poland are applied only for retail versions and description for those usually says that these can be activated in Poland, Czech Republic, Slovakia and Hungary. Correct me if I am wrong.

There are no Polish-locked gifts but there are Polish locked keys. They mainly come from CD Action Magazine.
Also there are cross-locks sometimes between Poland, Czech, Slovakia and Hungary.

Keys have much larger variance than gifts on locks, as publisher can create keys limited to any countries instead of four regions that gifts have.
