I would have said Steins;Gate but I've only read the VN and Fate Stay/ Night I have two more episodes to watch. Black Lagoon I'm on season 2. Then Black Butler, Death Note, One Punch Man, Tokyo Ghoul and Another are honorable mentions!

Unfortunately I don't hold control over the questions of the survey. XD You can choose series or movies or OVAs as long as it's anime. :3 I think a mixed top 5 is fine cause some people will already have a hard time listing a lot of anime.

Well, the purpose is to get the top 5 anime of the international anime community. "The best" is always going to be subjective no matter if you're asking a casual anime viewer or an experienced anime critic.
By keeping it vague and without specific years or categories, we see which anime have stood the test of time without limitation. By keeping it anonymous (which it will be, because the Google account is just to prevent duplicates), it ensures that no one's has to feel judged by which anime they choose.
By keeping it at 5 only, you have to decide: What anime do you love enough to list as part of your top 5?
Of course, I could just be over-justifying why the person who made the survey (it wasn't me) chose these kinds of questions. lol

Well, the great part about it is you don't have to justify why one is better than the other, or one anime is a choice to begin with. It's okay :) If it's a really great anime, more people will probably also list it, so I don't think you have to worry too much about it.

-- Cowboy Bebop (just great music, great animation and memorable characters)
-- Fullmetal Alchemist: Brotherhood (I like first filming too, but Brotherhood much more)

This form will be highly flawed in its final form.
The first step they should have taken is to have people find their favorite anime on either anidb or myanimelist.
Then they fill out the form with only numbers and what site they used.
That way there are no mistakes in spelling, and everyone knows what anime is chosen.

I don't control the survey form unfortunately, but for a user-centric design, I feel like it's pretty okay as it is. If you make the survey too complicated (e.g. with extra steps or with limited and difficult-to-navigate dropdowns), people (especially those answering on their mobile devices) are likely to just leave without answering. And you'll always remember the name of your favorite anime, even if it's a dub name, and Google will probably know the correct equivalent title.
