As far as i know "grad" means castle in Russian. It should be same for Serbian. And that's all i know about it... Maybe a native one can inform us!

Today, "grad" means city. Belgrade (Beograd) translates to White City. In the past, "grad" was used for a fortified town / fortress / castle, as you said. It does have a fortress with some pretty neat corners tucked away.
