Worst? I dug through a complete pack of Atari 2600 a while ago. Quite a few games made it to market despite barely being playable. There are definitely worse games on that console than ET. Custer's Revenge speaks for itself.
Most disappointing? Well, I'm pretty disappointed by the direction TF2 has taken in recent years, even though I don't play it any more. I don't even mind hats and microtransactions, but their approach to weapon and mode design has pretty much ruined all of the cool design decisions that made me like it in the first place.
Eeeeeasily Trigger Man, for the PS2. It's well renowned for having the worst [subjective, so lets go with 'one of the very worst'] combat systems ever in a video game.
To this day, the only other video game with a combat system that I've found completely bad was Witcher 1, and it still is far better than Trigger Man. :X
Last I saw, it was still rockin a solid 1.5/10 average review rating :P
And I'm pretty sure the people that voted it higher than 1, are the same people that vote bad games on Steam up :P

i own it, i even played it and its really bad.
the problem isn't the game itself/the gameplay.
its just really bad programmed and its too random.
If you know the crazy machines games: its kind of that genre, but you get different
results without changng your build. Its horrible difficult to get only Level solved, but i believe there are
not much people out there who gone through this cancer till the end and are still sane!
Gauntlet for NES was pretty terrible, it was one of those terrible arcade game ports which should have come with many quarters worth of lives, but only had 3.
