Since you've already received lots of other gifts, maybe something sentimental. Perhaps you could write a poem or set up a dinner that reflects all of the small details you remember about when you met or times you spent together. For example if she really loved Tetris as a child, this might be a cool gift. I think most people love to know you're paying attention to those seemingly insignificant facts about them.

I like this idea and do it periodically myself - regarding the 'small details' matter(s). It shows you DO pay attention...through tough and good times. It also keeps your mind active in this situation and helps both of you remember why you love each other.
I was going to suggest cooking a nice meal (if you don't cook often) - but I see many others have, so just consider another +1 for that option =p

I'd suggest a home cooked meal, it shows a lot more effort on your part than going out as well as the depth of your care for her plus you can make a romantic atmosphere as well.

I'm coming up to a decade and the best surprise I did for my wife was a surprise 'send her to a spa day' treat. I had a babysitter show up unexpected, surprised her with the spa day, lunch ,etc...
Now I'm not suggesting it has to be a spa day, but do something where you organize her day as a complete surprise to her. Maybe you say you're going to visit a museum and instead you drop her off somewhere she'd like to go. I really do think it was the fact that it was all a surprise and that I organized everything for her, that made it the most memorable thing I've done for her in ten years.
So seriously, do this, mucho nookie headed your way.

Sadly, she would probably be excited more by the museum and secondly, i'm a qualified massage therapist so she gets plenty of massages and spa days (at the spa and home). She really is spoilt now i think of it XD haha
