The gifter will contact you - usually on Steam or gift directly to you. Keep an eye on your email but if they gift directly through Steam you will receive a notification stating someone has gifted you a game. They have a week to send you the gift before you can report it as fake - but let us assume, for now, that it is genuine.

Correct, you must have the game in your inventory/extra copies list to be able to give the game away.
Once a game is owned and attached to an account as playable it cannot be gifted.

I don't think he got one. I'm pretty sure his friend created a private giveaway just for him, which he won of course. And they both expected the site would just automatically generate a free copy for him.
People are apparently endlessly surprised to discover publishers don't randomly give free copies of games to anyone who wants one.
