I think you have to ask the winner to write a message saying its OK to be deleted. Otherwise you have to deliver the game to winner (that he/she can claim) or get a "Not received".
Edit: And this was 2 keys in the GA. So both have to say its OK.

You would lose that slot for new GAs. But if you have enough, it doesn't matter.
But there is no check I know of for "not received" GAs. It can happen to you without your fault. You probably just shouldn't have a list full of "not received" GAs.

Well, 1 "not received" is nothing. It happens to people with working keys not redeemed, with keys that obviously were actually redeemed, with developers revoking keys, but the GA creator having bought them on a legit site ... And it can happen for a not declared region lock.
I would not recommend it for new SG users, but for someone with a couple of GAs that have gone through well, 1 "not received" is nothing.

You're right at some point, there are many reasons for a not received, there are cases that the giveaway creators can't do anything to fix. But there are cases the giveaway creators are just liars.
Anw my opinion is this: When you create a giveaway, your send a message/make a promise that you will deliver the winner a gift, and it's your responbility. And if you fail, you became the one broke that promise no matter what. I can't judge anyone when don't know anything about them, but the first feeling/impression when seeing someone like that is not really nice imho.

Phewww that's good to hear, yeah I got you. I said that for all honest giveaway creators too, in case my post made Irenie or someone feel bad like that. There are worst cases when the winner is an @sshole received the game but still mark as not received. That's why I said it's just the first impression, while we don't have much time to understand each others or find out the truth behind each not received mark. ( ͡~ ͜ʖ ͡°)

You loose a slot (as you do with deleting). If you have to many not recieved (looking like abuse) you could be punished.
Depending on which region the key is locked to I would try a reroll (is the chance high to get a winner that can redeem it or not. If you delete it you loose the CV for the already redeemed copy. CV is love, CV is life.

So, I created a GA for 2 keys and didn't see any information that the key was region locked, but one of the winners says they cannot activate due to a region lock. What's the best way to handle this? I'm think requesting a reroll (hoping it's someone who can redeem the key) and letting the original winner choose from a list of other keys I have (Humble keys, so hopefully not region locked but again I'd hate to run into the same problem) as an apology. Is there any standard protocol on issues like this?
