There are only a few games that I am aware of that still has functioning apps.

PS I'm talking about companion apps not apps that you NEED to use as it mimics a controller for the game and without it you can't play.

Mass Effect Andromeda : https://play.google.com/store/apps/details?id=com.ea.gp.masseffectandromedacompanion
Through this app you can send your strike team out to finish little missions which will reward you with materials, credits, weapons, and armors (potentially) you CAN access the missions in the game itself but its a lot easier to do it in the app whenever you're about.

Dying Light : https://play.google.com/store/apps/details?id=pl.techland.dyingLight.CA
Through this app you can send people (that aren't in the game or accessed through the game out on missions to get materials, money, ammo and some times weapons.

Fallout Pipboy : https://play.google.com/store/apps/details?id=com.bethsoft.falloutcompanionapp
Through this app you can access your ingame pipboy through your phone, this is good if you are playing with a mod that doesn't pause the game when you access your pipboy as you can just look at your phone and click on what you want to do. (Too bad it isn't compatible with Fallout 76 as it would be good for an online real-time no pause game to have such an app available.)

GTA 5 : https://play.google.com/store/apps/details?id=com.rockstargames.ifruit&hl=en_ZA
Through this app you can play minigames which will unlock ingame features.

No :(

Eon Altar requires each player to download the free Controller App to play, available on the App Store and Google Play.
Local Wifi is required to connect the Controller Apps to the Eon Altar core game. Internet Access is not required.

I have a fucking Windows Phone 10.

Same. Fuck these apps, though. The Windows Phone OS is great.

That isn't really a companion app though as you NEED it to play XD then we can just as well mention all the You Don't Know Jack games as I think they also use your phone as a controller

Well, I use Warframe app for some reason. I used to have the Neverwinter (mmo game) app, too. I guess some Assassins Creed games had their apps, too.

GTA5 has the iFruit app that allows you to play some minigames that unlock a few features in single player (for the dog, if I remember correctly) and in online you can use it to make custom license plates and I think in a certain measure mod a car.

Borderlands2 had an app, Loot the world, but I heard it got discontinued, you could scan random barcodes/QRcodes and you'd get random ingame weapons.

Far Cry 3 had an app, I think it is still functional, called Far Cry Outpost, where you could play a minigame and then get weapon mods/xp for the online mode. You could also customize your loadouts.

I was always curious about the Watch_dogs apps, I've only seen some trailers/quick presentations of people using it, but never really researched it further.

Just Dance on PC uses a phone app as a controller, for both the menu selection and to somehow track your movement ingame via probably the accelerometer.

Thief Town also has an app, that would allow more players to join and play, haven't tried it recently thou.

EXTRA: And pretty much all Jackbox Partypacks can use phones, but don't need dedicated apps, just a browser (so pretty much any device)

Cool had no idea GTA5 had such an app :) will add it to the list at the top, would be nice if the farcry and watchdogs apps were still running, but knowing Ubisoft it probably isnt

GTA app is usefull for colouring stock rims. :P For some reason you cannot paint default rims in GTA V, but I think they often look the best. The app is usefull as if you put whatever wheels on it and paint them, then put the stock rims back on through the app, they will be painted. :D

Pretty sure Battlefied has a companion app where you can change the loadout of your soldier.

I have used dying light app in the initial stages to get items but once you are half way through the game you generally don't need it as I played on hard difficulty bullets can't be bought on vendors so used the app to farm for bullets

Read that Ubi announced an Augmented Reality app for Division 2 but I don't know if it's really a companion app. More like a game adjacent app?

I like the Uncharted puzzle game, though I don't even own the console the main game is on. (I think it was for Uncharted 4?) The app can unlock some cosmetic DLC for the main game.
Not sure if you're looking at that kind of connection, or just gameplay type interaction.
