I'm baffled as much as you are. See for yourself here

EDIT: Seems to be released. Apart from the new achievements, I haven't found a changelog anywhere. If you have one, post in the thread and I'll add the link to OP.

EDIT2: Crystal Gamer listed out the changes here

Of course I do. With the new dlc what's the total of achievement you can get? I see 99 but i'm pretty sure it's the same as before.

99 with WotL & Eternal / Hardmode update. There is 15 new ones all with 'Eternal' in the name - things like Eternal God (RPG + Hardmode) and so on.

Try restarting Steam to see if it triggers the update?

I restarted steam and opened the game, only after that the update has come. Well, that's the first time it happens but thank for the help guys :)

It is very hard, Eternal Monsters and Bosses take a lot of hits and can corner you so easy. Rebirth seems so easy now.

There is no changelog on the Internet yet, but from what I've seen:

  • New "eternal" versions of bosses. There is still a chance to meet with old bosses on some levels.
  • Maximum amount of red hearts is increased. Now you can have up to 16 red hearts. The limit before update was 12 red hearts.
  • When you get more than 12 hearts the hearts tab zooms out. At first there are 2 rows with 6 hearts in each one (the same as before update), then 3 rows with 8 hearts in each one, then 4 (?) rows with 10 hearts in each one. I didn't see more yet :)
  • New type of room. I would call it Angel Challenge Room. It's similar to the Challenge Room but has the appearance of a Cathedral Room. Gives Angel Room items, eternal hearts.
  • Eternal hearts drop is increased in general.
  • Some new enemies (at least I didn't see them before). For example, big white spider which spawns ~10 usual spiders after being killed.
  • Eternal versions of some enemies (may be can be merged with previous point). For example, big white Boom Fly which booms for half the screen.
  • The pedestal (on which items appear) can be moved.
  • Planting bomb on Shift doesn't work now. Only E. May be can be changed in the settings
  • Less bugs.
  • Other less significant things that I forgot to mention

To add on:

*The item pool was modified. Here is a listing of some of the item pool changes: https://www.reddit.com/r/bindingofisaac/comments/34nekw/changes_to_the_item_poolsspecial_items_in_the_new/

*Samson was slightly buffed by virtue of Bloody Lust being made better.

*Luck stat affects D6 rerolls (negative luck increases the change of reroll into nothing (default is about 30%))

*Shot speed down pill exists (also lowers range)

Some of these eternal varients of enemies are really kicking my arse. Projectile dodging doesn't seem to be as much of an issue as the sheer amount of health they seem to have. Haven't tried with Eve yet, but considering how long it takes for Judas to kill one in the Caves, I worry about doing runs with her. Damage up items are going to be a necessity

well Judas, Maggy, and Isaac were extensively tested to have some sort of balance, and Samson was buffed a bit, But yeah Eve and Cain weren't really tested that much
