Hmmm thats a hard question, maybe an eagle so i could fly like Belmasv said, or maybe some aquatic animal like a dolphin, shark or a whale. Anyway it would be better to be a predator than a prey i guess.

A bird, just so I could fly. I would also want to retain my intellect so I know enough to avoid dangerous situations (like smashing myself into a window because I saw the sky in the reflection). I would also like to be able to revert back to my human self whenever I wanted to (of course, my clothes would transform with me, so I don't revert back totally naked).

A raven. They're smart, they can live for a few decades, and they can live pretty much anywhere.
