If you know when, then you can essentially say fuck it and do whatever in life knowing full well that nothing else could kill you.
Knowing how, well, you can take the steps to avoid ever being put in the situation where you will be killed by said thing and live and die of old age.
Having said that, knowing when is still more than likely better. No matter what, you're going to die after all.

I would choose how.
Death is inevitable, it will happen no matter what, so what the point knowing when?
But if you'll know the reason, you can try to cheat it, avoiding certain circumstances, or, if cause of your death is serious illness, even commiting suicide, right after diagnosis.

I'm not sure I would like to know either (or both), but it's interesting to note would you be able to do anything about it if you knew.
Purhaps if you knew how you would die, you'ld do something to prevent it, but would it be impossible or would you then realise how you would die then avoiding the previous "how"? Would it then be "an act of god", or a mortal choice (to go a bit religious). Also could you kill yourself before the time (or switch the method) is up?
