Since it's an English course, your friend might want to know that there are a few grammatical errors in the survey!
"Have you ever visited to Japan?" -> "Have you ever visited Japan?" (Or, "Have you ever been to Japan?")
"How many times have you ever been to Japan?" -> "How many times have you been to Japan?" (Same thing for "What cities have you been to?")
"Would you like to participate if you have the opportunity to cook Japanese food?" -> "If there was an opportunity to cook Japanese food, would you participate?" (Though this is kind of weird phrasing, since it implies a group event where everyone's cooking Japanese food?)
"Have you ever tried Strange Japanese food?(e.g. Natto, Raw egg)" -> "Have you ever tried strange Japanese foods? (e.g. Natto, raw egg)" (Capitals are tricky, I know.)

both of us are not native speakers so I didn't find these mistakes. when I received this questionnaire just answered it and shared link to lots forums and groups. Haha.
I'm Taiwanese and she's Japanese so I don't know what is strange Japanese foods actually means, too.

I was a bit baffled trying to decide what counted as "strange," too. I cook Japanese food pretty often, and I'm a big fan of e.g. konnyaku, which not many Westerners have tried. But I haven't had natto or raw eggs, so I just based my answer on that.
And it was my pleasure helping with the survey's grammar! I work as an English teacher, so it's actually harder for me to refrain from correcting things than to point out mistakes. :p

I have never been but I dropped you some answers to the rest.
One thing I'd like to point out is the description "strange" is quite vague and it can mean different things to different people. I can imagine a lot of people would already label sushi strange, especially with octopus or sea urchin. Best of luck to your friend, though.
