It surprises me that this hasn't been done before, but then again, it's just autohotkey with audio input support.

It has been done before. Windows Voice Recognition, for example. Voice Attack for another example. And many other smaller, less known voice recognition/voice control software packages.
This one and Voice Attack are just more specifically targeting gamers, not office workers, or home users with disabilities.
