A small LP Quiz

People made it clear to me that a few spoilers seems necessary, just so I don't waste your time:

  • This is a quiz, not a puzzle. There are no hidden cyphers or links or any sort of stuff like that.

  • The game is Rise of the Triad (the 2013 one)

A very small quiz.

Maybe it'll be a fun little journey of discovery for some of you. The prize is neither too cheap, nor too fancy.

Any feedback is appreciated (preferably spoiler-free, of course).

EDIT#1:
And what's a first ever puzzle without an ugly mess-up? FIXED QUESTION 2 SO THAT IT'S AN ACTUAL QUESTION! Sorry guys.

Tried to make Q3 a bit more clear, since it doesn't follow the pattern of the first two.
