I once changed it, tried to change it back to the default black square with a question mark, didn't work, so I just used a black square since then.

Very rarely. I changed my Steam avatar once to foil an impersonator using my steamtrades rep to scam people. (I know he could have just changed his too but I wasn't gonna make it easy for him.)

I know how you feel as I also have a hard time remebering some people and them changing their avatar sometime confuses me too.

Had an avatar for witcher 1. Never changed it until i got a pic from the art of witcher 2. When witcher 3 comes out, ill repeat.

Last time I changed my avatar was back in 02 or 03. Been using the same avatar ever since, everywhere. And people still can't figure out what it is :(
