How do we know if our account was affected, and is password reset necessary since they patched the security loophole?

If your account was affected Steam will be sending you an email with a new password and the recommendation that you change it immediately. This article covers it pretty well - http://www.gamespot.com/articles/steam-accounts-compromised-after-major-password-fl/1100-6429197/
