just only use the first box and you will be fine

Also if that is your real CC number it might be a good idea to remove the picture from your post.

Standard addresses are just whatever address you want to use for the place you are registering to, but Billing address = the address the credit card used has on file. Some (most?) credit card companies track using Postal Codes, and will deny charges if the address used for a purchase uses the incorrect postal code for billing address (hence many companies allow you to have a separate billing address).

Regarding the Address lines question:
Address Line 1 = Standard street address.
Address Line 2 = used for additions, if needed (such as apartment numbers and the like) You ignore this if you don't have an apartment number or something else that would go on Line 2.

Examples:
Standard address:
1234 Something Street
Sometown, SS
12345

Address with Line 2:
1234 Something Street
Apt. 2
Sometown, SS
12345

Only use Billing Adress (the 1st) and put there your adress.

First line: your home address.
Second line: if first line is insufficient, continue your home address there.
