"The Medic was planned to be a pre-order bonus to those that bought the game on Steam, however it is now available to everyone who buys the game on Steam, regardless of purchase date. Upon entering the Tavernkeep for the first time, there will be two crates containing your bonuses; one of these will contain the Medic (as well as the other three TF2 familiars)."

it "was" supposed to be a pre-order bonus. But everyone have it now >.<. Make's me feel like pre-order was not a good idea. also , they're not that good.

Simply cosmetic and they are somewhat useful at low levels if you haven't got any other pet of your level. Also, there's an achievement for all pets collected.

You get them once you first start single player or online in your own tavern and you see two crates that contains Tf2 pets

You need to have bought the game through Steam, not any other sites.
If you did buy it through steam, check the back/side of YOUR tavern near the stairs for two crates.

No.(.. but technically yes :P)
You just need to get the files for the DLC pets (which are empty(?) text files).

Yes, just having the empty text files in your folder, permanently unlocks the DLC pets/portal gun.
