Kingdoms of Amalur: Reckoning seems to fit your description. It is an awesome game, haven`t played it too much on steam, but have plenty of time on both ps3 and Origin. Absolutely worth buying.

Half-joking, half-serious post since you seem to be expecting open world games, but Dota 2 seems to kind of fit. :p
Everytime you try to explore the map, you'll meet an enemy hero and fight within 5 minutes.
Lots of action and panicking heroes wildly moving everywhere and blood everywhere during massive team fights.
Hundreds of unique abilities and item effects.
A ton of heroes.

Terraria may or may not fit this description. 2D gore FOR THE WIN!

I suggest Half-Life 2 and the first Bioshock.

Dark Messiah of might and magic is a great game. Has a great combat system and you can also do magic and has good gore

Witcher 3, if you haven't already played it. Only thing is that while it has a whole lot of action, it is also very story and dialogue heavy so if you are doing a main quest, in between the action you'll be having a few conversations. However the story is great and so are the characters, so you should enjoy it.
