I don't know (maybe 2 or 3 times), but I reached it today. :P Currently have 24 points.

Pretty much never. I try to keep my point total around 200, so I have a nice buffer in case some giveaways pop up I really don't want to miss. Sometimes I go on a spending binge and drop below 100, but I always keep some in reserve.

Happens quite a lot, but I leave a tab with a giveaway open auto-refreshing 'til I have the required points. :P

Rarely... that's because I enter only giveaways of games I want to play, not to fill my Steam games catalog
