Claimed means the purchase is associated with your HB account. When you make a normal purchase while you're logged in, it'll show up in your HB library and purchase history automatically.
But if you purchase w/o being logged in, or as a gift, you get the e-mail with the link to the keys but it does not go into your HB account unless you claim it.
It has nothing to do with whether the key was used/revealed etc. as far as I know.

"Unclaimed" just means that the purchase is not specifically tied to your Humble account and so the link is viewable by anyone that has it. Once it is claimed by someone it becomes private and can only be viewed when logged in with that particular account. It has nothing to do with whether a key is used or not. As stated above, this generally happens when you make a purchase while not logged in or if you received a gift.

For things that are in "unclaimed" status:
As far as gifts go, if an entire bundle is bought as gift then the recipient can resolve the above bullet points by simply claiming the purchase on their account. Note that checking the gift option when buying vs letting a regular purchase go into "unclaimed" status are pretty much the same thing.
In a case where an individual game from a bundle is converted into a gift link, there is no option to claim it for the recipient, so it is only accessible via direct link and will not show up in their Humble account. Only the activation key is provided and it will not yield the DRM-free version of the game or any downloadable content (if applicable) -- this will stay on the original purchase page (whether claimed or not).

No, it's a security bug on Humble Bundle site.
It's display as "Linked to YOUR_ACCOUNT", but this is not a true, because:

I can see how someone could confuse the whole "linked to [Account]" thing, but it doesn't actually have anything to do with the game or bundle in question and whether it is associated with your Humble account or not. All it means is that your Humble account is currently linked to the specified Steam account. That's it. It used to be used as a more user-friendly way of redeeming keys to a Steam account without having to do it through the Steam client interface (for those that didn't know how and had poor technical skills I guess) but I don't even know if it's relevant anymore.
P.S. oh and if someone else opens the gift link that you posted it will say "Linked to [their account]" (assuming they have this set in their options) so that serves as proof of what I said
