This is a rather coincidental story for myself tied in with the subject of this topic. For the longest time I had wishlisted Bloodrayne and its second game, but it never seemed to go on sale in the past few years. It was because of the sole fact that GOG still remembered it existed (and thus, had a discount sale price occasionally) that I had an account there in the first place. Then it evidently got a new publisher which also remembered it existed and had sales on it for every seasonal big Steam sale ever since.

Samurai Gunn is sitting cold and lonely waiting for its chance at being being bought. =P

Considering that H1Z1 slightly increases it's price before a sale, then during the sale they apply the discount to go back to its original price, you could say it was never on sale. So H1Z1.

Technically if it is on my wishlist, it hasn't been on a good enough sale for me to get it yet. But one really worth mentioning is:
The Walking Dead™: Survival Instinct. Apparently a trash game, but dammit I still want to play it because it has Merle in it. :D

Monsters Ate My Birthday Cake has never been on sale, and Elder Sign:Omens is occasionaly 25% off but never any more.

The Sniper Elite 3 devs seem determined to charge 65€/70USD base price for their 8 hour shooter.

I'm pretty sure Gridiron Solitaire has never been on sale. If it was it escaped me.

Darkest of Days, last time I remember seeing any kind of sale for it was way back in 2010 I think, if not longer.

Crayon Physics Deluxe, last time it was on sale was 2012 and I missed the bundles it was in 2-3 years ago. Same thing with Gish. :(

I was planning to pick up Telepath Tactics during the last two big sales, but the game seems so dead the developers don't even authorize discounts for it. Quite a shame.
