Two different names, two different ways of writing it. So none of your options is right for both.

It all depends.
The Dutch don't use a capital V for van. As you may notice Paul Van Dyk isn't Dutch but German and Van Helden is American.
Then there are Belgians who sometimes combine all words into one 'vanderUijle' and Americans/Canadians that capitalize or combine, like George Costanza in Seinfeld 'Vandelay' :P
