On Steam, of course

I haven't

Three times:
Once when I bought a game on sale and it went on an even better sale a couple of days later (already bought it as a gift at the better price to activate after being refunded).
Once when I bought a gift for a friend and they didn't want it.
Once when I bought a game that was bundled a couple of days later (bought the bundle and activated it).

According to Steam's terms of service, all of these are legitimate reasons, and I recently noted that I have not been put on the extended period until first card drops.

Not yet,The more costly ones have been good and some of the cheaper ones. For example less than a buck are. not worth fooling with and maybe will be fixed some day. some of them all ready have.

Just once on Steam. I bought a copy of some game (I don't even remember) a few days before it went on sale for a reduced price. I got my money back, and then I bought the game at the reduced price.

I haven't had to yet. I pretty much get all my games from either bundles or trading. When I DO buy a game directly from steam, or really anywhere outside of a bundle, I always wait for a steep discount and research the game itself for how enjoyable it is and how stable the build quality is.

Yes, because I forgot that I bought it elsewhere and just did not get around to entering the key via steam client.

I asked it once.

why? It was an online game but developers had it pretty much abandoned. It was dead and promises weren't delivered.
Did it work? No. I bought it many many many months ago due to my stupidity and a sweet deal. I couldn't play it but refunds din't exist at that time. They didn't accept it. Game is better now, they sold it, although I have no interest in it.

My bad, but it was wallet money.

I vaguely remembered one time, so searched my mails, and indeed in July 2012 I asked for a refund:

"I purchased the Double Fine Bundle, and when I confirmed I got a message that my PayPal payment failed and I was could use a different payment system. I assumed it was a momentary glitch, so backed and tried checking out again, which worked. Then I discovered that I bought the game twice, and have two copies of it in my inventory (I bought it to my inventory).

I would like to cancel one of these and get my money back. Thanks."

Said bundle went into my inventory, and was later gifted after it went on the bundle list.

Like 5 times. 1 time game turned out to be really REALLY bad, 1 time despite steam page saying I meet way over minimum requirements the game was running in 12 FPS even on lowest settings. Turned out that meeting way over lowest requirements mean I can run it on lowest in some shitty res like even below 720P -.-, 2 times game I bought for myself or for a gift during Steam Sale ended up being bundled short time later, so refunded them and bought it cheaper in bundle instead, and one time I bought a gift for group GA, but turned out not enough people were intrested in GA, so refunded gift and bought another one instead ;)

Fallout 4 - Got 60€ from a CS:GO skin, so decided to buy it. Couldn't run it. Now I can with mods, but I still refunded it, since I wasn't certain if I could run it. So, I'm entering GAs for it now :D
Creeper World 3 - Not sure why I refunded it tbh. I was afraid if it'll get repetitive, I think.
Battlefield: Bad Company 2 - It felt so awkward to play. The movement mechanics just sucked for me, plus the multiplayer was near dead.
Spore - 24 hrs after buying it, I immediately remembered how shallow the game was.

There are a few more, but I can't remember those ones.

I'd love to refund Rust, because it never ran for me. But.. alas, I couldn't. I bought it a long time ago for 20€. Probably just when it came out. It was supposed to run (back then, at least), but it never did.

Just once - the game seemed really interesting but I played like an hour of it and the mechanics were waaaay more boring than what I thought, and the story was so meh :/

Terraria because it was boring alone and I have no friends.
Lego Movie - Video Game because it was meh... It just copy and pasted the movie clips into it... Dunno... Other lego games have been better!

Once, game didn't work due to bugs.

But seriously, do you just sit there and think about the most random topics to post here ;D?

Once, for the choose-your-own-adventure ebook, The Sea Eternal.

I just wanted a widely-branching story about mermaids and sperm whales fighting giant squids. What I got was pretentiousness, railroading the reader up until the endings, cringeworthy preachy lectures on sjw stuff, and -- worst of all -- not a lot of the whole "war" or "branching" things that I'd bought it for in the first place.

No. Once I bought a bundle which said it will give you gifts of two earlier games and it didn't (I already had them) so I wrote to support and they offered to refund the entire bundle (because the wording was clear, which got changed later for that store page) but in the end I refused as that happened back when you unofficially had "one refund" and I didn't want to waste my one opportunity because it wasn't that expensive.

I haven't. and I hope that I never have to. but if tekken 7 turned out to use Denuvo crap, I'll probably have to do my first refund.

Yes, twice:
First one was due to game-stopping bug. I did not get the refund, as I had just crossed the 2h threshold when the bug showed up. And contacting the company that made the game and the publisher about it just resulted in them blaming each other for the issues (the game was Duck Tales)
Second one was because game (that somehow had great reviews) was bad. Hitboxes are not supposed to be so large that you get hit when you clearly are not touching the enemy... People praised it for its difficulty, but it mostly seem to have been down to hitboxes being the wrong size as far as I could tell... (the game was Castle In The Darkness)

Got quite a few tbh.

Europa Universalis IV DLC's: Reason? They went into a steam bundle and i missed that. So i bought em cheaper when the refund was done.
Rise of the tomb raider season pass: Was not supposed to buy it on steam but Kinguin.. Brain fart while buying a lot of stuff during sale.
Dreadout DLC's: I love both dreadout games but the dlc was money thrown in the garbage. You got close to nothing.
Tomb raider 2014 DLC's: They didn't show up as bundle when i wanted to get the last ones. But after quickly buying them 1 by 1 i looked up one of the few i didn't get and there was a bundle.. But it was not shown on the other DLC's for some reason.
Metro Redux bundle: Reason? I have no idea.. I even got all the games but it was on the list of games i refunded.
Day of defeat source: Reason? It's the COD of DOD games. I got it later in a trade but i can't stand it. The old one was better and now Day of infamy is far far superior.

Tried to refund.
Oblivion with all DLC's: Reason? Big shooping spree back in the days and i didn't even have it on my WL and for some reason it was added in the basekt. Never touched it and back then they told me to fuck off :/
Nether: Reason? Fuck those money hungry piece of shit devs and their abandoned game! Sadly this was before the refund part came to play.

Only once, when I accidentally added a couple of games to my account instead of to my inventory when making a gift purchase.

Torchlight 2
Bought 2 copies - one for myself, one as a gift to my friend b-day. Played +20 hours and all game-saves gone. Gift copy refunded (bought Titan Quest as a new gift) 😊
