Get outside. While in home, you'll feel like doing anything but studying. Going outside basically FORCES you to study, as there won't be anything else to do. Hopefully. :D

If you don't need your computer to study - turn it off. Turn your cell off too. And, as mentioned by Swordkill, try to study at another place, e.g. a library.

If you need your computer, close all possibilities of interaction (Steam, Skype, ...) and think about using a clean browser only to study, never to be used on steamgifts, youtube, ...

if you need to study using your pc i recommend you go to a public place or somewhere out. Because then you won't feel at ease playing video games or just browsing in general (to me it feels like someone is watching over my shoulder). Thank God i don't need my pc to study. I always study in my room and drink a lot of water so i don't forget to take breaks (pee breaks and food). I also leave my phone downstairs. I also summarize everything and sometimes i make cards for like maximal doses or structures i need to recognise.
And going to a library really isn't such a bad idea even though it's far. It means you'll think twice about wasting your time and your bed is so far away ;p

Chew gum while revising. Then chew the same type of gum during the exam. Your mind'll associate the gum with the topic. (This works with several things tbh; it's just a case of findwing what works for you... different flavours, different sweets/candies, different drinks...)
While revising, have classical music on in the background. Something slow and relaxing. It'll help.
Set breaks every hour or so; take a brief walk and allow your mind to slow down a little. If there's somethign you've had trouble with, let your mind mull over it during the break, but don't try and force it. Make the breaks for between 10 and 15 minutes.
Use muted lights. Nothing too harsh. If you've got something in a medium to dark green in colour to let your eyes rest on every now and then, it'll help. Tired eys will make it hard to keep on, rested eyes will make it easier, so every time you've studied a difficult part and your eyes and head are aching, go look at something green and have a glass of water. It'll help.

In order to read, it's important for me to make breaks during reading, in order to avoid thinking that what I'm doing is boring. And try to reward yourself with something whenever you read a specific amount of pages. ;)

It's basically a to-do list with dailies etc. You can use it for every part of your life.
It has a lvl system, skins, stats, community features and so on.. basically a to-do list that motivates you with RPG elements :)

I barely use my phone nowadays, so if it's on or off it really doesn't make a difference. :D

Never heard of Habitica, but it sounds really cool so I'll definitely give it a go at least when I'm starting university, might not have the adequate amount of time right now to design such a thing.

Taking breaks is not a bad thing. You just need to limit how long the breaks last. Five minutes is good for a "casual" break. Other breaks, like the one you take for a meal, will be longer. It is normal for a student to take a break for 5-10 minutes after ever half-hour of studying.
More important, however, is making sure you get enough sleep. Yes, you can study and memorize and practice, but it is only when you sleep that your brain organizes and sorts and files that information away. If you are studying hard, you will need a nap in the middle of the day and good sleep at night. The more you study, the more sleep you will need for your brain to sort things out. Without enough sleep, your brain will have trouble remembering and using what you've learned.

So yeah, I'm basically starting my exams in exactly two weeks and I could do with a few tips as to how to stay focused during studying. I mean, I can revise, but I always seem to get distracted after 20-25 minutes. God damn it YouTube! :D
