Are there many ultra-realistic shooters, simulation, etc.?

ArmA, Flashpoint (I think that's what it is...) the older Rainbow Six games. America's Army is pretty good... That's all I can name off the top of my head.

Edit: Operation: Flashpoint: Dragon Rising is what I was thinking of. Some of the people that worked on ArmA worked on it.

Hmmm I think the most realistic one right now is modern warfare 3, their melee system is pretty good!

The Fallout series are pretty realistic.

You know what's a pretty good one? It's probably not what you have in mind, but... SWAT 4. It's a lot of fun, and realistic enough to suit.

Flashpoints from Codemasters are arcade shit. Old "Flashpoint cold war crisis" from BI is now called "Arma cold war assault".

I've always been kinda disturbed when people say they want a realistic shooter. "It's not real enough." What do you want to do? Actually shoot someone and watch them die? Geez people.
