Neo: I thought it wasn't real
Morpheus: Your mind makes it real
Neo: If you're killed in the matrix, you die here?
Morpheus: The body cannot live without the mind
So given the explanation above, what happens when you get killed in the matrix and it happens too fast for your mind to know about it... such as meteor landing on you or you get shot in the head from behind? Inquiring minds need to know.

Also my guess is that if you get killed in the Matrix, you're brain might shutdown or literally kill you. Your brain controls all of your bodies functions and could turn off functions of your body or simulate pain by itself. It would likely shut down major organs or functions that would result in death. Kind of like being a vegetable.

I don't think it's a matter of the mind knowing you died, but a form of consciousness. Do remember that their bodies always bleed when they're heavily injured in the Matrix. So I'd say it's the brain not knowing what to do, and if a sudden death happened, I guess it would just be like an heart-attack or something fast like that.

This is one of the most bullshit explanations in an otherwise great movie. It's one thing if the brain believes the body has suffered a trauma and starts flooding the system with adrenaline (or whatever it tells the body to do when suffering severe physical damage), but bullet holes that appear out of nowhere in 'real life' because of been shot at in the matrix is another and is plain retarded.
They could have went with better explanations, such as that the machines unplug the body when it's should be dead in the matrix to keep it believable (but then that would mean that logging into the matrix under human controlled platform would make you immune to death in the matrix so I guess that's a not a good theory).
I tell myself that's death in the matrix invokes neural shocks to the brain (or whatever) delivered by the giant needle you have to stick in your head in order to log in. This "feature" could be a so fundamental and core mechanic of the matrix that human engineers and programmers didn't find a way to get inside the matrix with that feature disabled. This way the whole matrix concept as presented in the films feel more right to me. Hope that helps you too.
Oh and I almost forgot, thanks for the giveaway!
