Can they region lock an item after you've added it to your library, or do anything else at that point?

I think so, I believe this happened with Borderlands 2.
Can't be completely sure.

I can confirm it didn't happen with Borderlands 2

It was region locked, but any Russian versions in libraries (such as mine) were still completely fine :)

That wasn't the point I was trying to make though, it was the fact that they could modify the game once the person has activated it.

I knew there was game that gave gave a region-locked version, and because of the outcry customers a ROW version was later added, but I just wasn't sure if it was Borderlands 2 (which it turned out to be).

Yes. RU+CIS Thief was region locked after it was added to library by retroactively adding in an onlyallowrun tag that would not allow those outside of the region to play the game. XCOM: EU also was locked temporarily after the fact as well as Aliens: Colonial Marines.
