Done! No "Folk" option and I listen to pretty much any genre, so I just put "Other".

Question 11 needs an 'Other' answer. There are more online music stores than you listed, and the example list for Websites implies pirating music rather than purchasing it from legitimate sites.

some suggestions

q3 - way too few. add the option for 'i listen to a variety of genres/eclectic'
q4 - remove 'other' and make 'least favourite from those listed' and add 'none of the above'
q5 - download? for free legal, free illegal or paid legal?
q7 - could add something here for people that don't have parents, like myself, to feel a bit better.
q11 - websites = juno / bandcamp / beatport... but you make it about piracy.. you ONLY list itunes, which do not sell much lossless.... this question is messed up, do I answer 'websites' for juno, or 'itunes' (when I would NEVER even HAVE an appleID or touch itunes 256 mp3 or the odd ALAC if you're lucky to find it)..

you don't mention other formats. CDS? what about cassettes, minidiscs, vinyl, 8 tracks...

you don't have any real options concerning free netlabels.

I can't afford to give you guys a giveaway at this time because i am going out of the country on a trip for 3 weeks so i may just create a quiz for some DoTa 2 invites or just post them here! anyways thank you guys so much once again! You may continue to answer because my results will be submitted tomorrow afternoon the more results the merrier :)

I just took the survey and agree with other posters here that it's too restrictive on the allowed answers. As an example: I don't really acquire new music anymore. I just listen to stuff I downloaded years ago. Unfortunately there's an implicit assumption that the survey taker is actively seeking out new music and buying/downloading it.
