It will be activated at the same time you activate Orange box. You should get a "proof of purchase" hat when you startup TF2 next time

Go to your inventory and click add game to game library.

Yes you do. Did you ever read the TF2 FAQ? Anyways any purchase of TF2 in retail or in a Steam pack will give you premium(ONCE, duh)

It's been said many times, but yes. Check your steam history for confirmation: It should list "Team Fortress 2 - Premium DLC" among the other games that Orange Box activated.
