OK finally solved. My favorite science fiction book is probably The Three Stigmata of Palmer Eldritch. My favorite sci-fi game would be the original Half-Life. And my favorite sci-fi movie would be the original Blade Runner.

Favorite SF book is a toss up between Dune and Neuromancer.
Favorite show is Battlestar Galactica.
Favorite game is Mass Effect (I view the trilogy as a whole, but if I have to pick one then 2).
I feel like there have been so many good SF movies that it's hard for me to pick one, but let's say The Fifth Element because I've loved that movie since it came out but for completely different reasons throughout the years.
Also, your puzzle is quite the puzzle. The very first clue has me stumped.

That took a bit of brute force finagling on 3 and 5 due to the number of possibilities....
It's hard to pick a favorite so I'll go with some goodies that I haven't seen mentioned yet.
Books: The Postman by David Brin, also his New Uplift trilogy (the original is good too, but the new one is a much more coherent epic space opera). Also I found a new (to me) publisher Angry Robot who's been putting out some quality sci-fi. Check out the Tao series by Wesley Chu. Mort(e) by Robert Repino is a fun take on post-apocalyptic wars, I'm assuming that everyone is also already aware of Robopocalypse and it's follow-ups.
Games: try Bioforge, it's got cyber raptors, beating someone to death with their own arm, you know things for the whole family. Giants: citizen Kabuto has a great sense of humor with it. Sunder from the recent humble monthly is good too, and CryptArk (I've been playing roguelikes a lot because I'm stuck on a potato).
TV Series: Just started up on Happy and liking it so far. Made in Abyss is a good anime. Add in Orphan Black, Warehouse 13, Fringe, and Red Dwarf for some variety and to cover all your SF-trope bases.
Movies: Screamers - campy sci-fi fun. FAQs about time travel, Sunshine, District 9, What happened to Monday?

Above all the great movies out there, my favourite SF is the original Star Wars trilogy and don't think that will ever change. XD
StarGate SG1 and Battlestar Galactica were my favourite SF TV shows.
I read fantasy books as opposed to Sci-fi, so can't name a book in this case.

Very hard to pick my favorite scifi book because I've read (and re-read) so many but any top list would have to include More than Human by Theodore Sturgeon.

Favorite SF tv show: The Twilight Zone
