Instructions:

Find a PNG hat template with google search.
Download GIMP, its free (Paint and Photoshop is good enough too)
Open your avatar with GIMP
Open your hat as a Layer
Correct the position of hat
Export image
Upload it to your steam profile
Wait few hours until it appears in SG (no need to sync)
