2012, the one that looks like the mamba althought im pretty sure both are good. I am using the Mamba 2012 and is a great mouse. Never cared for wireless but it is a great experience. Battery is rechargable and you can always connect the cable to use it will charging if need it (haven´t need to do that yet). It is a tad expensive and you pay a premium for being Razer but for what i can tell is worth it. The only thing is you need to install the drivers to work correctly.

I just picked the G500 up when it was on sale on newegg for $30 off and would happily pay full price for it if I had to. I have fallen absolutely in love with this mouse. It feels fantastic in the hand. The texture on the sides give you a great grip, even on a hot day during the long grind sessions. The custom weights give you plenty of options to get the glide just how you like it. The on the fly dpi adjustment up to 5k is wonderful as well, though it does mean you lose two buttons if you don't hotkey it to a keyboard combo and just use it on the mouse. I haven't found a single fault with it yet. The laser is super sharp, and tracking is incredibly smooth.

Logitech g700 best mouse i ever had i do so much work on pc and this is the best mouse i ever had, okay cost´s about 50-70 buck´s but dude how often did u buy a mouse?? every year or every second year? so its nothing

the gigabyte m8000 is much better than that garbage 6k model, and depending on your budget something like the gigabyte avia is actually pretty good
if that's not your thing though then I would suggest the newer genius gila
I use the m8000 mentioned above and have for a couple years and at this point basically if it ever broke I'd probably just replace it with the same thing or the gold edition of it, mostly just because I don't need wireless and 3 profiles is enough for me.
The gila though has my attention and I plan on getting one with my next build just to check it out.
all three are laser, and I think the gila is actually dual laser, I forget?

I use the Razer Naga Hex. I like it a lot, specially for MOBA games like LoL or ARPG games like Torchlight or Diablo. I highly recommend it.

I like the g9x. DPI adjustment is right below the left click, which is awesome if you ever play FPS games with sniper scopes. Yes, it has proprietary software, but that's just for macros and LED color, the DPI adjustment buttons work without it. 70 bucks for the regular version, or the MW3 stickered version for 40. It's what I use daily on my gaming PC. Fits every one of your criteria and is old enough to be cheap.

Buy the new Logitech G Series, seems they are very good and cool. I personally have Razer Deathadder, is very good mouse slightly weighted, but in future I will buy a Mad Catz RAT Cyborg or a Logitech G Series they're more comfortable to my hand, but more expensive too >.<

I currently use a Habu Laser Gaming Mouse. The buttons started wearing out and double clicking with single presses. I cleaned them. The left one works, but now the right won't click at all. The metal piece was warped inside the micro switch. I order some replacements switches, but also decided to look for a better mouse. For the time being, I have a side button mapped as the right button.
I tried the Gigabyte GM-M6900 Precision Optical Gaming Mouse and found its tracking to be too rough for even causal usage. I attribute this to the fact that it is optical. I am returning it.
I also tried the SteelSeries Sensei Laser Gaming Mouse. It tracked much better, but I found the forced usage of proprietary software too burdensome for the price. The two right buttons just make the mouse freeze and after a little waiting it reconnects. I use GNU/Linux and hate it when i have to use Windows to configure a mouse. Since this mouse was supposed to save the settings, I thought it might be okay. To my dismay, even on Windows this mouse was a nightmare to configure. The software crashed repeatedly and I was unable to configure it. I found several other similar complaints. I am returning it.
Any suggestions? I want a mouse that doesn't rely on required proprietary software to work, scroll wheel with button (practically a standard now days), has at least two side buttons, prefer adjustable DPI, and is a laser mouse.
