If both have keys then use other tradable games as a deposit. Person A will give tradable game to a middleman. Then person B give key to A. Then A gives B key. After that A gets his tradable game back.
If A doesn't give B his key then middleman can give B the tradable game.

OR Middleman is actually a scammer friend of B and unfrieds A as soon as he gets the tradeable game.

One thing for sure, just spend the time to check out the person you are dealing with and if you have to deal with middlemen, the middleman too. Trusted ones are marked on steamrep, so copying the person's profile link into steamrep search bar on your own (to prevent impersonator's fake links) more or less means you are safe.
One thing about Case 2 with canceled Steam purchase, if A does that, his profile will be suspended. So it's unlikely, unless the game has been bought with a new account no one cares about.

There is nothing you can do with regards to getting anything back. Steam probably wont do anything. The only thing you can do is submit your evidence to Steamrep, like I did when I was scammed http://forums.steamrep.com/threads/report-76561198014484240-pizdos-skutena-balvonas.1644/

I'm confused as to how :) Surely it means whoever purchases the key can turn it directly into their inventory so that people can see they actually own said item and it will be visible in a trade window rather than not knowing if the key will even work. Item trades also come up in trade history so you have indisputable proof that the trade happened and it is logged by steam.
It'd also solve the problem I've had in the past of not knowing if a key has been redeemed or not. (ones of my own when clearing out old bundles and such) :)

Exactly, i almost always have traded by going first, but pretty much only with high rep profiles. Never had a problem.
If a lower rep wants to trade, he must go first, or i just skip that trade. Also in high value trades, i never trade with low rep, guys with nothing to lose, or if something just doesn't feel right.

To me it sounds like either you have been a victim of an impersonator (If you were shown a ST or similar sites profile) or simply the user had a bunch of fake profile rep.
New created accounts with lots of rep in a short period of time are likely to be scammers.

I'm sorry to hear this. Next time try to be more careful, I would even recommend trying to avoid trading keys unless you're completely sure you can trust the other part or you have a trusted middleman in the transaction. Trading keys is always risky, that is how most people get scammed. :<

srly being scamed if your own fault, if you throughly research the person you are trading with, profile, games, inventory, it is public?, steamrep, check if he is giving fake credentials (like claiming he got alot rep on steamtrades but the steamprofile there is not the same), etc.
I have been active trading from some time and now scammers add me on a daily basis and its way too easy to identify them. anyway if in doubt never go first or just do not trade keys which goes to your own risk based on steam TOS.
Also if you think being scammed is the only problem of trading keys them you are mistaken, you can get banned from steam if you get a key gotten by fraudulent methods.
