Hmm, I remember playing this one a long time ago (stopped halfway for no reason) and I heard great things about this one. And that's pretty much all the games I know from that bunch.

Main factor as it changes anything, or main factor as has to be gay? Most RPGs that have homo and hetero romance options do that (like Mass Effect, Dragon Age, even Stardew Valley) by not defining the orientation of the protagonist, and just letting you choose. (For these I think you could go for some LGBT friendly curator, or store tag)
Your Dry Delight is a free to play VN with a gay detective, and Dream Daddy is about a gay dad dating other gay dads. I'm sure there are more "canonically gay" games, but these I could remember right now.

I think KotOR is the first one I played that allowed for that although I guess Mass Effect and Dragon Age are the ones most "famous" for it because it got discussed a lot when they were released.
At this point it feels like it should be normalized, especially since it's optional and no one needs to play in a way that makes them feel uncomfortable (for some reason) but what do I know?

I´d say Rimworld qualifies.
It´s available on Steam and other stores. Pawns, as the characters are called, have a sexual orientation and as Pawns in a relationship are generally happier it becomes a factor. A major factor !? Idk, there are a lot of factors in Rimworld ^_°

I'm not sure if it's what you're looking for, but the main character in Hades is bisexual and you have an option to date a guy. You also have other LGBT characters, and it all feels very normalized. It's also a fantastic game

It's post-game content, you need to develop a bond with a few characters. 2 women and 1 man are dateable, and you can date all of them at once with no repercussions, but also choose just one

Everyone knows Tacoma (https://store.steampowered.com/app/343860/Tacoma/) and Gone Home (https://store.steampowered.com/app/232430/Gone_Home/), which are walking sims, if you like this genre these games are very good.
There is the criminaly overlooked The MISSING: J.J. Macfield and the Island of Memories (https://store.steampowered.com/app/842910/The_MISSING_JJ_Macfield_and_the_Island_of_Memories/), which is an excellent dark puzzle-platformer with very serious writing.
There is Ikenfell (https://store.steampowered.com/app/854940/Ikenfell/) which is a JRPG with fun combat, cute characters, but some of the writing is a bit juvenile. I recommend it, though.
Also, there is Lucah: Born of a Dream (https://store.steampowered.com/app/896460/Lucah_Born_of_a_Dream/) which is a soulslike with very smooth gameplay and while the graphics aren't the prettiest, the game has distinct identity and it never bothered me in my playthrough. Great game.

To be honest, there was nothing that could have told us this was a joke.
In this case, I think it's got less to do with people's level of sensitivity and more with your ability (or inability) to properly convey a certain meaning.

The protagonist of Red Strings Club is gay but you can never find out. VA11 Hall-A has a lesbian protagonist and her ex-girlfriend is somewhat important to the plot. In Hades the protagonist is openly bisexual and there are gay characters, but in ancient times homosexuality was not a big deal so it's just like any other couple.
I mean, those romances are important, but they are treated just like any other romance would. There are no conflicts involving self discovery or homophobia, so I don't know if this is the kind of stuff you are looking for.

in Cyberpunk 2077 you can chosse the sexual orientation of your character.
in Iconoclast there Is a secundary character that Is lesbian, dosnt have any sexual act, and i dont wanna give a spoiler of the ending, about the part of this especific character. (its a great game btw, recomended to play it!)

While one could argue that it's implied they're gay, it isn't explicitly mentioned in the game that either (the main antagonist or king) are. Regardlessly, I don't feel it's a major theme in the game. You must be quite mad to claim GK 3 aged well :D Anyway, anyone nostalgic is unfit to judge whether a game has aged well or not. No offense is intended however, I just wanted to voice my opinion on the game, others may disagree.

Also to add to this: anyone who's interested in Gabriel Knight 2 would also probably like Phantasmagoria 2.
It also fits into the theme of the thread because the main character is revealed to be bisexual in part of the story, and his sexuality is a subplot within the story.

The protagonist in Pendula Swing was married to a woman. There is a part of the story that is about overcoming her grief.
I haven't gotten far enough in the game to confirm it due to technical issues but I believe there are romantic options later on.

The title made me think you want to know if I was looking for Gay-themed games (I was ready to answer "only if they are good"), but the post seems to indicate you are looking for them. That question mark, I guess. :D
You really mean ANYTHING at all? I suppose there is lots of stuff of all qualities and tastes -- quick porn, elaborate stories, RPGs where being gay is an option (often important). But when you say "sexual orientation of the character", this already seems to exclude some things (like, jigsaw puzzles with gay porn content? those do not even have a main character). Is it because you want to play them, or for research, or a gift...?
Anyway, while I do not think I am an expert on Gay-themed games nor on Visual Novels (I played only a few so far), I loved Highway Blossoms, I would recommend it; actually, maybe you should check VNDB -- they have (nearly) every VN ever released, and they are well tagged so you can filter to fit what you are looking for.

So, something where being gay is an "integral" part of the story, not an option, so to speak? Ok, I think Highway Blossoms still qualifies! But I am sure people will come up with better examples.
I also remember reading a series of well written articles which mentioned many games with gay themes, more towards the sex side... hold on...ah, here: part 1, part 2 and part 3.

I am more looking for games where the sexual orientation of the main character is either the sole focus of the game, or a major part of it. For example a dating Sim like Coming Out on Top is all about gay dating so it fits my requirements, where as Mass Effect, where you can choose any orientation, but it has no significant impact on the overall story, doesn't fit my requirements.
