I distance myself from my point of procrastination by finding something else to distract me. Games keeping my attention? Switch to Youtube. Youtube being too attention needy? Browse social media. Getting too deep in the blackhole which is social sites? Gaming time.
But more seriously, the best way I find myself able to focus is not start distracting myself ot begin with, which is tough. I have trouble getting back on track after losing my focus, so I try to avoid the addictive nature of websites and videos and games ENTIRELY until the assignment is done, because usually it's very hard to get back on track.

I once took a "little" break from doing this one assignment and then the last thing I remember is I failed the subject :(

If I am procrastinating when I have something important to do, I eliminate all other stimulus until the only thing I have left is the task at hand. Sometimes this isn't possible, but the closer you get the better and the effort involved can be good to shift momentum away from dumb shit and into the to-do list.
If I'm procrastinating because I'm bored with unimportant stuff I'm doing, then I just let it be. It's OK to let your mind wander sometimes, and there's no fulfillment in forcing yourself to carry on with things that don't actually matter. You'll be fine if you stop watching that series to go play a game, or stop that long game to go read a book, etc. etc. etc., or to stop whatever to just sit and think a while.

will creating a to-do list do the trick?
like that feeling you get when you got to check/strike-through lists (I did try making one but then confused about what should I write, didn't work out so well)

To-do lists can be great, but they can be disheartening too. Be careful in constructing them such that they contain only things that are achievable on whatever timescale the list is intended for. Something sitting on an actual list forever (or getting copied endlessly onto new lists) can make people sometimes procrastinate more. If you have impossible items or items that are too abstract, get rid of them. Then sort your long term stuff into a long term list, and your short term stuff into a short term list, etc. Break it down however you like from there, if needed.
