Secret of Mana (SNES)

DBZ Budokai 3 - Finished about 10x , Deleting the save and doing it again.

Bioshock Infinite, GTA San Andreas

Baldur's gate 2, total annihilation, Starcraft, Warcraft 2, Lords of Magic, Tiberian Sun. All 10+ times.

Should be Serious Sam: The First Encounter, multiplayer coop of whole campaign... countless times.

Final Fantasy 7 on PS1 takes the cake for me.

I play all of these at least once a year, sometimes twice.

Skyrim, DAO, Sanitarium, The Longest Journey, Dreamfall: TLJ, Grim Fandango.
Couldn't even guess how many times I've played Sanitarium, TLJ and Grim but there's no way I've played each of them separately under 50 times O.o
DAO and Dreamfall: TLJ probably around 10 times
As for Skyrim, I often start with a new char, play way too much hours in one session and then have to turn it off because I get addicted. xD
Can't even try to guess how many chars I've played with but I distinctly remember 5 with whom I've finished the main storyline and a whole lot of side quests.

Diablo 2 or Pokemon Fire Red.

Original Rome Total War, in the middle of a Scipii campaign at the moment. Actually have been playing it a lot over the past few weeks.

I shudder to think what my overall game time is. Luckily, I mostly play it off of Steam.

Replayed Chrono Trigger on New Game+ so many times all my characters had maxed out stats. That was long ago.

Replayed a lot of Final Fantasy VIII too. I think Rinoa has the best limit break ever out of all FFs.

There are other games I spent more time on, but I don't count them as "replays".

Completed Jak 3 on my PS2 about 6 times, I think it's just a great game, it felt like a completely new experience everytime, there was just so much unique locations and mechanics. I really wish they would make another one, but I think the story is finished.

Fallout 2. several complete plays. amazing game. every small choice matters. you can't make it the same twice

That is a long time of playing DA:O (19 times, wow). I remember Orzammar got boring as fuck at some point, I just wanted to get out. :p
I am not one of those persons that replays games much, my most replayed ones are mass effect 2 and dragon ages , about 3 times each? That is a lot for me. :p

I dont really replay games nowadays tbh .
Back in the day i guess like 90% of my console games , mostly the final fantasy games on the PS , and bunch of even older ( super mario ... the classic 1 , Tanks ... excite bike ... etc )

The only PC game i can say i replayed alot would be the Original Starcraft and brood war i guess .
I replayed the campaigns several times , and thats kinda the only game i replayed more then once .
