so i brought isaac dlc for my cousin and he got it but when he tries to play its just normal? any ideas??

its activated, don't bother with the title.

You won't see any changes unless you actually play it.

As much as I heard you only notice the new content if you keep playing, new weapons appear, a new ending, a new charakter etc.

Nope. There should be big "Wrath of the Lamb" title in the main menu now.

the starting screen is old isaac it should be the wrath of lamb

Right click binding of isaac > Properties > DLC. If you don't see Wrath of the Lamb there, it''s not installed, if you do, stop whining and play the game

The 1.3x (which introduced the stuff that's in Wrath of Lamb...I think) is mandatory, everyone got it even if they didn't own the DLC. I suggest playing the game to unlock new items to see if that works.

Has he played the game yet? I'd do at least one dungeon run then restart Binding of Isaac.
