Try setting default ultra then high then medium setting and see if fps and visuals get better , then experiment
Also have you considered the video card either not doing it for witcher 3 or maybe malfuction?

How did that videocard not melt instantly when you start up the game for the first time?
Minimum requirement to play witchyman according to steam is Graphics: Nvidia GPU GeForce GTX 660 / AMD GPU Radeon HD 7870
Radeon 7870 is 15~20% better than 7850 if you look at the fps in most games that they tested back then in 2012.
Spend 100$ (or probably less secondhand) on something like a Radeon R7 370 4GB (if you want to stick with AMD) and you can paint for trololo who guards boats so nicely in a shocking 45fps on medium settings.

Witcher 3 is extremely well optimized and can be played well below minimum requirements. Devs often put out higher than acyual min requirements to be 100% sure game with run without problems for all ppl owning such hardware, it often does not mean game cannot be played with weaker hardware.
I played W3 on PC even weaker than OP's - I have old i5 650, GPU is HD 6850 with only 1GB of VRAM and still game played well ;)

what resolution? I started at 1080p and had 20-24fps as well, but after changing to 900p it was already 45+ fps most of the time (with ocassional dops to 3X) and game was still very enjoyable at this res. And at 720p I had stable 60+ fps.
Generally most of games can be played with older hardware on lower resolutions - if options modifying does not help enough there is also often hidden config options which may help. I recently finished Prey (the 2017 game, not the old one), much newer game, much more demanding than Witcher 3, and with a little bit of config magic it ran at 900p at stable 50+FPS (and it didn't look bad at all, you can see at screenie below). Only moment you cannot really do anything is when game is using some technology that is present in modern GPUs only and depends only on that technology (there is no option to use older less demanding and less pretty tech). So far I only found one game I was not able to tweak into playable state because of that - The Division. Other than that I tweaked a lot of modern games and they all worked - Witcher 3, Rise of Tomb Raider, DOOM, Prey, Total War: Warhammer, XCOM 2, Mass Effect: Andromeda, Batman: Arkham Knight, Dark Souls III, Metal Gear Solid V, No Man's Sky, probably more I don't remember now, all way below minimum specs yet all very much playable ;)

Btw, I tried Witcher 3 before on 7650 HD and it worked. Not great, but it worked, as you said - 20-ish fps.

I am playing at 1366x768. My monitor wouldn't let me experiment with your theory even if I did think of it, but that is a wonderful idea! I commend you for that! ;)
Welp, that's a thing you learn having a Pentium 3 with a GeForce 2 while almost everybody else has cards with pixel shaders and all them new games comin out askin for them - tweakin tweakin tweakin :D

not only that, when I switched to 900P I could even boost some options to medium and still keep it stable and playable ;)
EDIT nvm, In read claimed instead of chimed ;p Also Witcher 3 is nothing - I recently finished Prey (2017) - much newer and more demanding game, 50+ FPS at 900p and it didn't look half bad ;) But for this one in-game options were not enough, had to tweak config file a little bit ;p

Try putting the default LOWEST settings then walk around a little and check your fps, then take it to default MEDIUM (or even HIGH?) setting and check the performance again. Once you hit a lower than desired FPS (either on average or minimum) start taking down some of the most demanding graphical options.

I wouldn't say it's the resolution - you have quite a few demanding settings turned on and quite a few on medium/high ;) I played in 900p as well, on weaker GPU and had over 45 FPS most of the time with only ocassional drops to 3X on special maps with loads of particle effects (not sure how far in the game you are so not gopnna say which ones cause "no spoilers" for this masterpiece ;p).

Is the game supposed to look like that? Looks somewhat cartoonish, most obvious on sunsets. Also shadows appear pixelated as if they are made of black mesh, don't know how to explain it.. Antialiasing doesn't fix it, and drops me quite a bit of fps. Using 7850 HD 2gb from Asus, newest 17.6.2 driver
EDIT: the problem is already fixed thanks to user named Mully (lowering down the sharpening)
