What are the repercussions for refusing to send a game to a winner and having them mark it Not Received? Do you just lose 1 giveaway slot and that's it? Is the number of times you've done this tallied somewhere? If you did this like 50+ times would there still be no other repercussions?

You get a suspension for not sending a win IF you do it with the intention to prevent that someone get his win, without that he broke a rule.
It's something different when you have a faulty key and can't replace it etc.. In such a case you don't get a suspension on top to the lost 1 GA slot and the stat change (+1 not sent game) in the profile.

Withholding a win is not cool.
Having a faulty key and unable (or reasonably unwilling- massive sale, or bundle ended) to replace is tolerated, accidents happen.
I'm fairly sure the mods keep note of these events, either actively, or when they notice it being too common.

Nothing of this is quantified, it's mostly on expecting good will and honesty, as my experience goes here.

As masafor and adam said - suspension if done on purpose, and withholding a win is not cool.
Enjoy the blacklist (unless of course you had a legitimate reason, which you haven't cared to share.)

Pretty sure it's just a -1 to a giveaway slot unless it was done with malice and the giver didn't like the winner.
I only ever had one game I couldn't replace. A revoked key, and I'm pretty sure the other person was a bot as responding to anything was above their capabilities.
