I can find things wrong with every game I've played. The closest thing I've played to perfection though? Diablo II (with the expansion of course).
I think the more important question is, "What makes a perfect game?" My answer:

My criteria: Memorable art and design, memorable and engaging story (if applicable), memorable characters (if applicable), memorable music, intriguing gameplay, at least decent-to-good sound design, a satisfying ending (if applicable), and (possibly post-patching) essentially zero bugs (sometimes bugs themselves can be endearing, so I don't necessarily count them against a game).
So, I'd +1 Grim Fandango.

I think that the original Plants vs Zombies was a near-flawless execution. It does so much right, and the only thing it does wrong in my opinion is unnecessarily requiring you to click on the suns to collect them - pointing at them should be enough.

No such thing. But the ones that came closest for me were Return to Castle Wolfenstein, Half Life 2, World of Goo and Bioshock(the first one, not Infinite, although that one's awesome too, don't get me wrong).
