Basically anything from Monster Hunter series. Gotta fix my PSP screen to play it once more.

I find SMB to be kinda easy, comepared to other games. I love MegaMan series though. Still can't finish the first game, dammit :(

La-Mulana. I recently got to Hell Temple in the remake.

They Bleed Pixels is pretty damn hard. Similar gameplay to SMB, but won't compare their difficulty directly, haven't touched SMB for too long to do so accurately. But TBP gets my vote.

Ghosts'n Goblins series are my favorite games, i like the second game Ghouls'n Ghosts the most.

SpaceChem was the first thing that sprang to mind, but your list reminded me of Contra. Contra it is.

Gothic 1 and 2 were unbeatable without "god" cheat when I was young. Now, there is countless amount of games that are too hard. Mostly these are come side-scrollers, platformers e.g. The Binding of Isaac, BEAT TRIP BEAT/RUNNER. But the hardest are Terraria and Garry's Mod :D Truly, the hardest AAA title is Dark Souls (this poison when you didn't add any points to the poison resistant.

Battletoads was incredibly hard, especially the sewer jumping level. That level barely gave you time to blink, let alone reflexively move and jump. And if you failed, you'd have to beat the first few levels over again. It wasn't very fun.

One game that hasn't been mentioned yet that is awesome (but also ridiculously hard) is Magician Lord. That level is as far as I could ever get, but being a Magician/Ninja/Robot Dragon makes it amazing.

Super Meat Boy

Teenage Mutant Ninja Turtles (NES). I never got past the 4th level where you had a gta type game play and rand through the sewers. I have nightmares still over what might have been the next level while still fondly remember the joy of randomly saving my fellow turtles on rooftops.
