I can find things wrong with every game I've played. The closest thing I've played to perfection though? Diablo II (with the expansion of course).

I think the more important question is, "What makes a perfect game?" My answer:

  • Designed to be exciting for AT LEAST 500 hours. My biggest problem with games today is that developers want to design for players who play for 30 or 40 hours. I want a game that draws me in for a thousand (or the estimated 5000+ hours I put into Diablo II over the years).
  • Infinite potential for improvement. I want to get attached to my character. I don't want to hit a level cap, have all the best unique gear and be bored because the only thing left to do is go play a noob again. I don't want that ever.
  • PVP with rewards and CONSEQUENCES. I used to play this MUD, where everyone was able to be PK'd within certain rules (most relevant was that you could be attacked at any time by any player who was 1.5x your level or less... and that was for softcore players. Hardcore meant you could be attacked anywhere at any time by anyone). When you died, you lost 4 or 5 percent of your total EXP (this loss WAS capable of deleveling you). Yes, this meant there were dicks and griefers, but those dicks and griefers get powerful by being the best. With that said...
  • Rewards for being good at the game. Don't give me an achievement badge for beating the main quest and be done with it. Give me badges for completing challenges that maybe 1% of the playerbase will ever even try. Hell, give me actual, tangible rewards for that. If I'm awesome at the game, give me somewhat overpowered shit that signifies such.
  • Metagaming. Give me opportunities to play other players just as much as I play the game. Don't make special items untradeable. Make players keep themselves well informed or risk losing them to a clever salesman. Another example: In the aforementioned MUD, I kept multiple characters at various levels, mostly well geared, so I could PK enemies at any level. Then, when guilds were created, I essentially formed a mafia, offering protection from my many alts (I was also the highest level character in the game at the time), as well as training in the mechanics of the game's PK system (there were EXTENSIVE helpfiles, but they were, well, extensive, and most players didn't want to read 30 pages to figure out 4 commands) in exchange for A: Agreeing never to PK fellow guild members and B: Using your new PK knowledge to hunt down non-guildies at every opportunity (Sadly, PK eventually got nerfed due to whiners, or I'd probably still be playing that MUD).
  • Of course the standards apply too -- balance, lack of bugs, interesting mechanics, unique skills, distinguishable classes, etc. However I think the above are my biggest gripes, things that I find most games to be lacking.

A game that is "perfect" to me is my most favorite game of all time.

The name of that game is: Minecraft. I could write a wall of text why after 3 years I still love that game, but I won't.

My criteria: Memorable art and design, memorable and engaging story (if applicable), memorable characters (if applicable), memorable music, intriguing gameplay, at least decent-to-good sound design, a satisfying ending (if applicable), and (possibly post-patching) essentially zero bugs (sometimes bugs themselves can be endearing, so I don't necessarily count them against a game).
So, I'd +1 Grim Fandango.

I think that the original Plants vs Zombies was a near-flawless execution. It does so much right, and the only thing it does wrong in my opinion is unnecessarily requiring you to click on the suns to collect them - pointing at them should be enough.

No such thing. But the ones that came closest for me were Return to Castle Wolfenstein, Half Life 2, World of Goo and Bioshock(the first one, not Infinite, although that one's awesome too, don't get me wrong).
