Super Ghouls n Ghosts, large portions of my childhood were ruined to getting the fairy ring ending,

Hardest game EVER: Volgarr the Viking. If you ever beat it, I will kiss your feet. xD I couldn't even beat the first level and I'm not really someone who can't beat games. :P

Gothic 2 night of the raven. Super Meat boy.
Yeah, dark souls is hard, but its basically trying and "git gud" over your mistakes. Game is easy once you know "things". A game when one mistake is heavily punished can't be considered actually "hard" because it was YOUR mistake. However, I undestand why the community (and myself too) consider Dark Souls a hard game.

Hm, I'll keep it to Steam Games. As achievement hunter a lot of games can become stupidly difficult. But if you are asking for normal playthough only:

I wanna be the guy. It's not for Steam but you can play it for free. Makes some games like Super Meat Boy to look like a playground.
Honorable mention for Steam: 1001 Spikes. It will test everybody's patience for good.

Among 'the newer games' I would just say Dark Souls like most have already said. But I think that there were harder games amongst Commodore 64 / NES games. On C64 the difficulty usually came with the thing that you couldn't save, savepoints were too far away or you just didn't know what to do and there weren't any guides unless you bought game magazines like ZZap!64 (also included a c-cassette which had some game demos), Mikrobitti (magazine here in Finland, usually had great hints like the protip here)
Then there were the Ultima series. Oh boy I had some great time with those games even though I didn't really play them through most of the time. But you can have great time flying with hovercard and shooting bears with lasers (Yep, first Ultima was a bit scifi compared to the sequels). Also Castle Master was quite hard back then but had really great music. Aaaaand my first game Monty on the Run, never completed nor knew what items to choose in the menu and what could I do with them.
Just to say it short: There have always been difficult games but I think that Dark Souls is the most known of them. I think that the reason is that the development team just thought that we want to make sequel to the Demon Souls and make a hard game. Nothing else matters.

Probably Dark Souls 2. Not that it was better than the first, but because some of the design decisions made the build I chose more difficult than I expected, and some of the mechanical changes made it more difficult to break out of my muscle memory.
