The only people who've blacklisted me did so when I expressed that I didn't like or trust Epic games, and wished people wouldn't support them as they are bad for us as gamers. The reaction was largely "but mah free gamez!1!``!!" and several blacklists. So, while I'm not sure why I'd blacklist someone, the general answer to the original question, in my experience is that people blacklist if you have an opinion they disagree with.

I don't blacklist people in general,even though since I started been active again here I got quite a few blacklists myself which I don't mind. But I find petty to blacklist someone just because you don't like his avatar,different oppnion, ratio or having thousands of games in your library...

People get blacklisted for pretty much everything you can imagine. If you are part of SG Support, that in itself is enough to earn a blacklisting. On a personal level, I do not mind if others blacklist me. Blacklisting is a function of our site, and our users should feel free to use it as they wish. I do find it a humorous, however, that some of the people who have me on their blacklist will take me off it long enough to enter my giveaways. And then after the giveaways are over, they add me back, again. D
To return to your question, I almost never use my blacklist. A user has to be uncommonly toxic and/or harmful to our SG community---with no sign of remorse---for me to add him or her to my blacklist. To give you some context, most of the people on my blacklist are permabanned from the site.

I never blacklisted anyone as far as I can remember, never seen a reason to do so.
I'm also one of those who question the mentality of BL someone for not holding giveaways, it's quite absurd imo since those "requirements" would make this a site for tradeaways and not giveaways.
If I give something (be it on SG, Reddit, Discord or elsewhere) I never expect anything in return since it's a gift.
Edit: also, BL is completely broken since people who BL you can still enter your GAs, very strange indeed.

I have only one on my blacklist : a guy who won one of my GAs, and never activated it after one week despite being "online" all the time, never answering to my messages. So re-rolled, and blacklisted that bot.
I know i have been blacklisted by someone on a GA train, because i had made a little mess (that i helped resolve though). The other 10 i have no idea.
I know some people told me from the orphan keys topic that they were blacklisted after winning a key while somone else tried to get it 2 seconds later.
People can be so fragile in their heads sometimes...

Other people do it for all manner of reasons. I'm sure people have given you many reasons: saying thankyou, not saying thankyou, not giving away enough, giving away the wrong thing, winning one of their giveaways, etc. As long as you are visible, you will be blacklisted. You will probably be blacklisted just for posting this question. By the same token, some people also seem to whitelist for all manner of reasons. Try not to lose sleep over it because it is not possible to participate on this site not be blacklisted.

I tend to only blacklist people who are excessively rude, or thier opinions are being forced on others or are extremely [insertprefixhere]phobic / [insertprefixhere]ist.
I also on very rare ocassion blacklist those who have won more than they have given, but it has to be a huuuge amount, even then sometimes I don't (maybe depends on if they say thanks or not.)
I have experienced people blacklisting me for the following:
Difference in opinions aren't bad when they're not detrimental to a certain group of people. Those who I have blacklisted have/had opinions of that manner.
Addition: I saw something that Khalaq said that was also fitting that I forgot about, "with no sign of remorse", that and also devoid of either compassion or comprimise.

Just wondering, tbh. I started being a little bit more active on SG these last few weeks and was surprised that I got blacklisted twice yesterday/today.
I usually blacklist only scammers (only have 2 people on BL), so I was curious about which other reasons you would blacklist someone.
